Beyond Chin Fillers
Chin Augmentation
Dr. Jonathan Zelken, MD · Board-Certified · Newport Beach, CA
One of Dr. Zelken's favorite operations, chin augmentation adds height, width, or projection to the chin. It takes little time yet creates meaningful, lasting changes to your profile and facial balance.
Approach
Dr. Zelken's Philosophy
A beautiful chin is never about chasing a trend — it is about balance. At the Zelken Institute for Aesthetic Medicine in Newport Beach, board-certified plastic surgeon Dr. Jonathan Zelken approaches chin augmentation as one piece of the larger picture of facial harmony, refining the profile so the chin sits in natural proportion to the nose, lips, and jawline.
As both an artist and a scientist, Dr. Zelken believes moderation is essential. A chin that is too large can masculinize a face or draw attention to itself; the goal is a result that looks entirely your own — refined, balanced, and undetectable. Every plan begins with Vectra imaging and an in-person evaluation, so your implant is chosen to complement your unique anatomy rather than force a one-size-fits-all ideal.
Because he performs facial procedures every week, Dr. Zelken has refined each step of the operation to be efficient, safe, and precise — delivering meaningful change through one of the quieter, more elegant procedures in facial plastic surgery.
The Procedure
What is Chin Augmentation?
Chin augmentation adds height, width, and projection to the chin — an important component of facial balancing and a harmonious profile. It is often performed alongside rhinoplasty, facelift, submental liposuction, or buccal fat removal, but it works beautifully as a standalone procedure as well. There are several variables to consider: width, height, projection, implant material, and incision placement.
The dimensions and shape of the implant are guided by your existing anatomy and should reflect your unique goals. To plan precisely, we use Vectra 3D imaging to simulate the shape and size of your result before surgery, so you can see the change and refine it together with Dr. Zelken.
A helpful rule of thumb is to draw an imaginary line between the nose and lips and evaluate how the chin relates to it. The classic "ideal" comes from historical studies and artistic masterpieces and naturally varies by ethnicity, trend, and individual taste — which is exactly why an in-person consultation and Vectra analysis are so important.
Materials & Approach
About Chin Implants
Whatever the material or route, the fundamentals stay the same: a safe pocket against the jawbone, and a device delivered symmetrically for a natural result.
- Silicone Implants Dr. Zelken most often uses silicone — a firm, malleable material, not a soft gel — from two or three trusted manufacturers, matched to your desired width, height, and projection.
- Porous Polyethylene Implants can also be made of porous polyethylene. Both materials have comparable, well-established safety profiles; the right choice is guided by your anatomy and goals.
- Submental Incision Made in the natural crease beneath the chin, avoiding the mouth. It leaves a small scar — often as little as one centimeter — that Dr. Zelken keeps inconspicuous.
- Intraoral Incision Placed inside the mouth for no visible scar. Because it passes through a non-sterile space, it may carry slightly higher risk; Dr. Zelken will discuss which route suits you best.
- Precise, Symmetric Placement A pocket is created safely against the jawbone and the implant is delivered symmetrically, so the result is balanced, stable, and natural.

Is It Right For You
Who is a Good Candidate?
Good candidates for chin augmentation are healthy adults who feel their chin is weak, recessed, or out of proportion with the rest of the face. Many come to us after years of chin fillers, ready for a permanent, one-time solution rather than repeated maintenance.
You may be an ideal candidate if you are bothered by a soft or undefined jawline, want a stronger profile, or are already planning a rhinoplasty, facelift, or neck procedure and want the chin to complement those results. Non-smokers with realistic expectations tend to do best.
Because the goal is balance rather than a dramatic or trend-driven change, the best way to know if the procedure is right for you is an in-person consultation with Vectra imaging, where Dr. Zelken can evaluate your anatomy and preview your potential result together with you.
What to Expect
The Procedure
Your journey begins with a consultation and Vectra imaging, in person or virtually, where Dr. Zelken evaluates your profile and helps you choose the right implant. The surgery itself is brief and performed in a fully accredited facility with board-certified anesthesia. A small, carefully placed incision — beneath the chin or inside the mouth — allows the implant to be positioned precisely against the jawbone.
Healing Timeline
Recovery
First Few Days
- Expect mild swelling and tenderness around the chin, easily managed with the medication provided.
- Most discomfort is minimal, and a supportive dressing or tape may be worn briefly.
One to Two Weeks
- Swelling steadily subsides and most patients return to work and light social activity.
- Any external sutures are typically removed within the first week.
Four to Six Weeks
- Residual swelling continues to resolve and your new profile settles into its final, natural shape.
- Most patients resume full exercise around this point, per Dr. Zelken's guidance.
Long-Term
- The implant becomes a permanent, natural-feeling part of your profile.
- The improved balance of your chin and jawline endures for years to come.
The Outcome
Results & Longevity
A chin implant delivers a permanent, stable result — no touch-ups, no repeat appointments. Once healed, the implant feels like a natural part of your face, strengthening a weak chin and bringing the profile into balance with the nose, lips, and jawline.
Satisfaction rates with chin augmentation are very high, in large part because the change is meaningful yet subtle: a stronger, more harmonious profile that still looks unmistakably like you. And although an implant is designed to be permanent, one of its quiet advantages is reversibility — if you ever wished to change course, the implant can be removed or exchanged.
As with all of Dr. Zelken's work, the aim is a refined, undetectable outcome — noticed as an improvement, never as surgery.

Risks & Considerations
As with any surgery, chin augmentation carries some risk — including cosmetic dissatisfaction, infection, bleeding, visible scarring, or temporary changes to sensation or smile. Fortunately, these complications are very rare. And because an implant can always be removed or adjusted, the procedure is more forgiving than it may seem. Dr. Zelken reviews every consideration openly during your consultation and provides detailed pre- and post-operative guidance.

Common Questions
Chin Augmentation FAQ
Answers to the questions Dr. Zelken hears most often about chin augmentation.
Yes — a chin implant is designed to be a permanent result. If you ever wanted a change, though, it can be removed or exchanged, which makes the procedure surprisingly forgiving.
Implants are made of silicone or porous polyethylene. Both have well-established safety profiles; Dr. Zelken most often uses silicone from a few trusted manufacturers.
Either beneath the chin, in a natural crease, or inside the mouth for no visible scar. Dr. Zelken will recommend the safest approach for you.
With the submental approach, the scar is tiny — sometimes just one centimeter — and placed in a natural crease, where Dr. Zelken keeps it inconspicuous.
Fillers are temporary and need repeating every year or so. An implant is a one-time, permanent solution — ideal if you're tired of maintaining filler results.
Yes. Chin augmentation pairs naturally with rhinoplasty, facelift, submental liposuction, or buccal fat removal for a balanced, harmonious profile.
They can. An overly large chin may masculinize or unbalance a face, so Dr. Zelken emphasizes moderation and uses Vectra imaging to plan a natural result.
